To that end, we have made some changes to the manner in which patient calls and in-person appointments are handled, effective immediately.
YOU WILL BE ASKED THESE PRE-SCREENING QUESTIONS BOTH IN PERSON AND OVER THE PHONE*:
1. Have you had any known exposure to, or contact with, any individual with a confirmed case of COVID-19/Coronavirus?
2. Have you or anyone in your household traveled to any area of high incidence of COVID-19/Coronavirus, either within the United States or abroad?
3. Do you have any of the following symptoms?
- Fever of 100.4 or higher
- Shortness of breath
- Sore throat
*IF YOU ANSWER YES TO ANY OF THESE QUESTIONS, IT IS EXTREMELY IMPORTANT THAT YOU CALL OUR OFFICE AND LET US KNOW SO THAT WE CAN DISCUSS ALTERNATIVE VISIT OPTIONS, SUCH AS A PHONE VISIT OR A TELEMEDICINE VISIT.
